8. Date of Appraisal
9. The appraised FMV (Fair Market Value) of the vehicle
10. The Method of valuation used to determine FMV
11. The Specific basis of the appraisal, including comparable vehicles.

To process an appraisal we need:

1. Filled out questionnaire
2. 10 Pictures of the vehicle
3. $150 and payable online.
